Petsmart.com has traditional snap-on nylon collars with a personalization option for a name, and I think even your phone number. If not, locate an embroidery shop in your area and take it to them. They embroider on hats, backpacks, sports equipment and stuff like that, so a dog collar isn’t probably to far out.
